AbstractObjective：Thisarticleaims toexplore theimpactsofdexmedetomidinecombinedwithsevoflurane-basedintravenousinhalationalbalancedanesthesiaonostoperativesleepqualityanddeliuminlderlypatientsundergoinglaparoscopiccolorctal surgery.Methods：Atotalof106elderlypatientsundergoinglaparoscopiccolorectalsurgerywereenrolledfromZhangzhouHospitalAflitedtoFujianMedicalUniversitybetweenJune2O24andJuly2O25.Theyweredividedintotwogroupswith53patientsin each：thecontrol groupreceivedroutine anesthesia，whiletheobservation groupreceivedsevoflurane-basedanesthesiaonthebasis of theroutineanesthesiaregimenadministeredtothecontrolgroup.Afteranesthesiainduction，thefolowing indicatorswerecomparedbetweenthetwogroups：emodynamicparametersatdiferenttimepoints，incidenceofpostoperativedelirium，wakening qualityidicatorsand postoperativesleepquality.Results：Afteranesthesiaadministration，themean arterial pressure（MAP）and heartrate（HR）of thestudygroupattimepoints T1，T2，T3，and T4 were notablylowerthanthoseofthecontrolgroup，withll differences reaching statistical significance（ P<0.05 ）. Additionally，the study group exhibited the significantly lower incidence of postoperative delirium compared to the control group （ P<0.05 ）. In terms of recovery indicators，the observation group had markedlyshorterspontaneousbreathingrecoverytime，extubationtime，andeye-openingtimethantheontrol group，andalltese intergroup differences were statistically significant （ P<0.05 ）.Furthermore，the postoperative Pittsburgh Sleep Quality Index （PSQI） scores of the study group were significantly lower than those of the control group（ P<0.05 ） . Conclusion： Dexmedetomidinecombined with sevofluraneanesthesiacaneffectivelystabilizethehemodynamic statusofelderlypatientsundergoinglaparoscopiccolorectalsurgery，educetheincidenceofpostoperativedeliium，andimprovethequalityofawakeningandpostoprative sleep.
