Social Work Intervention in Case Management for Residents with Mental Disorders at F Welfare Home: A Practice Study from a Collaborative Perspective

Abstract: In the context of the National Action for the Integration of Community Rehabilitation Services for Persons with Mental Disorders, social welfare institutions urgently need to address challenges in mental health services. Based on qualitative interviews at F Welfare Home, this paper examines issues such as insufficient collaboration, lack of professionalism, and weak psychological support. From a collaborative perspective, it employs case management methods to optimize the service chain Evidence shows that social work helps diversify management approaches, enrich mental health services, and strengthen home-facility coordination, thereby enhancing service effectiveness and individual self-efficacy.
