Influencing Factors of Systemic Inflammatory Response Syndrome in Patients with Type 2 Diabetes Mellitus Complicated with Upper Urinary Tract Calculi after Transurethral Flexible Ureteroscopic Lithotripsy

Department of Urology，AnhaiHospital， Jinjiang City，Quanzhou 3622Oo，Fujian，China [Abstract] Objective To analyze the influencing factors ofsystemic inflammatory response syndrome （SIRS） in patientswith type2diabetesmelltuscomplicatedwith upperurinary tractcalculi after transurethralflexibleureteroscopic lithotripsy.Methods From January 2023 to September 2025，94 patients with type2 diabetes melitus complicated with upperurinary tractcalculi who were treatedinAnhai Hospitalof Jinjiang City were selected.All patients underwent transurethralflexibleureteroscopic lithotripsy.According towhetherSIRSoccurrd，theyweredivided into 25 cases in the SIRS group and 69 cases in the non-SIRS group.The diffrences between groups were analyzed，and theinfluencing factors of SIRS inpostoperative patients were exploredby multivariate binary Logistic regression analysis.Results There were significant diferences infasting plasma glucose，2-hour postprandial plasma glucose，preoperative urine culture positive，preoperative serum creatinine，operation time，intraoperative pyuria and postoperative white blood cell count between the two groups （all P<0.05 ）.Multivariate binary Logistic regression analysis showed that elevated 2-hour postprandial plasma glucose，preoperative urine culture positive，elevated preoperative serum creatinine，longoperation time，intraoperativepyuriaand high postoperative whitebloodcellcountwereindependentrisk factors for postoperative SIRS （all P<0.05 ）. Conclusion Elevated 2-hour postprandial plasma glucose，preoperative urine culture positive，elevated preoperativeserum creatinine，long operation time，intraoperative pyuria and high postoperative white blood cellcount are independent risk factors for postoperative SIRS in patients with type2 diabetes mellitus complicated with upper urinary tractcalculi.The results of this study can provide evidence-based basis for perioperative management of patients.
