Clinical EffectofNoninvasive Positive PressureVentilation inthe Treatment of Elderly Patients with Diabetes Mellitus Complicated with Severe RespiratoryFailure

HUANG Tianhang，SHI Rongze，WANG Jianming Intensive CareUnit，Quanzhou FirstHospital， Quanzhou 362Ooo，Fujian，China [Abstract] Objective To explore the effectof noninvasive positive pressure ventilation in the treatmentof elderly patientswith diabetesmelituscomplicatedwithsevererespiratoryfailure.MethodsAtotalof78elderlypatientswith diabetesmelitus complicatedwith severe respiratoryfailure admited tothe IntensiveCare Unitof QuanzhouFirst Hospital from July 2O23 to July 2O24 were selected and divided into control group （ n =39，drug treatment）and study group（ n =39，drug treatment combined with noninvasive positive pressre ventilation）according to the treatment plan. The clinical indexes，lung function，blood gas indexes and incidence of adverse reactions were compared between the two groups.Results The timeof blood glucosecontrol to normal，the timeof dyspnea disappearance，the time of asthma disappearance and the time ofhospitalization inthe study group were shorter than those in the control group， and the differenceswere statistically significant （all P<0.05 ）.The forced expiratory volume in the first second，forced vitalcapacityandratioofthetwointhestudygroup wererelativelyhigh，andthediferences werestatisticallsignificant （all P <0.05）.ThepHandarterial partial pressure of oxygen inthe study group were relatively higher，and thearterial partial pressure ofcarbon dioxide was relatively lower，the differences were statistically significant （all P <0.05）. Theincidence of adverse reactions in thestudy group was lowerthan thatin the control group，the diference was statisticallysignificant（ P<0.05 ）.Conclusion Noninvasive positive pressure ventilation in elderly patients with diabetes melitus complicated with severe respiratory failure can improve their blood gas index and lung function，accelerate the improvement of clinical symptoms，and have high safety.
