[Abstract] Objective To investigate thechanges inserum levelsofapolipoproteinA1（ApoA1），apolipoproteinB （ApoB），andglycated hemoglobinA1c（HbAlc）inelderlypatientswithtype2diabetesmelitus （T2DM）andtheir clinical ignificance.Methods Theclinical dataof10O T2DMpatientsadmited tothe Third People's Hospitalof Lanzhou from March 2O22 to March 2O25 were retrospectively selected.The patients were divided into the poor prognosis group （29 cases）and the good prognosis group（71cases）according to their prognosis.Thelipid and blood glucose indicators of the two groups were compared.The independent influencing factors of T2DM prognosis were analyzed using multivariate binary Logistic regression analysis.The predictive value of serum ApoA1，ApoB，and HbAlc levels for the prognosis of T2DM patients was evaluated by drawing thereceiver operating characteristic （ROC）curve. Results The fasting blood glucose，ApoB，and HbAlc levels inthe poorprognosis group were higherthan those inthe good prognosis group，whileApoA1 was lowerthanthatinthe goodprognosis group，the differences were statistically significant （all P <0.05）.The results of multivariate binary Logistic regression analysis showed that ApoA1（OR=0.686，95% CI： 0.505-0.932），ApoB（OR=2.745，95% CI： 1.872-4.025），and HbA1c （OR=2.941，95% CI： 1.961-4.411） were independent influencing factors for the prognosis of elderly T2DM patients （all P <0.05）. The ROC curve showed that the combined detection hada higher area under thecurve than the detection of serumApoA1，ApoB，and HbAlc alone.Conclusion Serum ApoB and HbA1c were expressed at high levels in elderly T2DM patients with poor prognosis，while ApoA1 was expressed atlow levels.Combined detection can effctively predictthe prognosis of patients.
