Protective effect of selective renal artery branch occlusion on postoperative renal function in robot-assisted partial nephrectomy

AbstractObjective:Toanalyzetheprotectiveefectofselectiverenalarterybranchocclusiononpostoperativerenalfunctionin robot-asisted partialnephrectomy(RAPN).Methods:76kidneycancerpatients whounderwentsurgical reatmentatMiyun Branch, PekingUniversityFirst Hospitalfrom May2O23toMay2O25wereincluded.Theywererandomlydividedintotheobservationgroup (n=38,receivingseectivernalatrybrancholusion)andthecotrolgoup(n=38,receivingompleteolusionoftemaieal artery)usingarandom numbertable.Allpatientswerefollowedupfor2months.Theperioperativeindicators,dynamicchangesof postoperativerenalfunction,renalfunctionijury,andcomplicationrates werecomparedbetwenthetwogroups.Results:Terewere nostatisticallysigificantdierenesinoperativetime,traopeativebloodlos,andtransfusionateetweenthetwogroups( P >0.05). Thewarmischemiatimeandpostoperativelengthofhospitalstayinteobservationgroupweresignificantlyshorterthantosenthe control group ( P <0.05).Compared withthecontrol group,theserumcreatinine (Scr)levelsof theobservationgroup were lower,and the estimated glomerular filtrationrate (eGFR)levels were higherat1 day,7days,1 month,and2 months after surgery P<0.05 ). Repeated measures ANOVA showed that the group effect,time effect,and interaction effect were all significant ( P <0.001). The incidence of acute kidney njury(AKI)andtheprogresionrateofchronickidneydisease(CKD)intheobservationgroupwerelowerthanthoseinthe control group (P<0.05 ).There was no statistically significant diference in the total complication rates between the two groups ( P >0.05). Conclusion:SelectiverenalarterybranchocclusioninRAPNcansignificantlyshortenthewarmischemia time,reducepostoperative renalfunctionijurypromotenalfunctionrecoeryandlowertheisksofAKIandCDprogressionhichisorthyfootio.
