Application of CBL Combined with PDCA in Ultrasound TI-RADS Training

Abstract: Objective To improve ultrasound physicians’proficiency in Thyroid Imaging Reporting and Data System (TI-RADS) through the training mode combining cased-based learming (CBL) and PDCA cycle based on the rising detection rate of thyroid nodules and the suboptimal application of TI-RADS by ultrasound practitioners. Methods A training program,including TI-RADS theoretical lectures, stratified case reviews,pathology-based discussions,and clinical practice feedback,was conducted in the Ultrasound Department of Shaanxi Provincial People’s Hospital from September to November 2O23.Pre- and posttraining assessments included theoretical examinations,case interpretation accuracy,and clinical application metrics.Results A total of 5O ultrasound physicians completed the training.The accuracy of benign and malignant thyroid nodule differentiation improved significantly (57.6% vs. 63.3% , P=0.003 ),and TIRADS 4a classification accuracy increased from 38.0% to 58.0% ( P<0.001 ). The proportion of Category 4 and 4a nodules in ultrasound reports decreased markedly,while malignancy detection rates via contrastenhanced ultrasound 60.0% vs. 82.0% , P<0.001 )and fine-needle aspiration biopsy (80.0% vs. (2 90.0% , P<0.001 ) rose substantially. Conclusion The training mode of CBL integrated with PDCA cycle effectively enhances ultrasound physicians’mastery of TI-RADS.
