TheImpact of SGLT-2 Inhibitor Dapagliflozin on Glycemic Fluctuations and Cardiovascular Risk Indicators in Elderly Outpatients with Type 2 DiabetesMellitus

[Abstract] Objective Toanalyze theefectof sodium-glucosecotransporter-2（SGLT-2）inhibitordapagliflozinon glycemicfluctuationsand cardiovascular risk indicators inelderly outpatients with type2diabetes melitus （T2DM）. MethodsFromJanuary 2O22 to January 2O25，8Oelderlypatients with T2DMwhoreceived treatmentat the Department of Outpatient in Jinan Sixth Retired Cadre Sanatorium of Shandong Military Region were conveniently selected. They were divided into groupsaccording to diffrent treatment regimens.Both groups received vasodilator，diuretic， cardiacstrengthening and hypoglycemic treatments.Onthis basis，thecontrol group（4O cases）was treated with sacubitril/valsartan，andthestudy group（4Ocases）was treated withdapagliflozin inaddition tothecontrol group.Theblood glucose indicators，glycemic fluctuations，cardiovascular risk indicators andadversereactions of thetwo groups were compared.Results After 6 monthsof treatment，the fasting plasma glucose，2-hour postprandial plasma glucose，glycated hemoglobinA1c，largestamplitudeofglycemicexcursion，standarddeviationofblood glucose，postprandial glucose excursion，meanamplitudeof glycemic excursion，mean ofdailydifferences，C-reactive protein，serum uricacid， triglycerides，total cholesterol，diastolic blood pressure，andsystolic blood pressure levelsinthe studygroup were lower thanthoseinthe same group before treatmentand thecontrol group，andthediferences were statisticalysignificant （all P<0.05 ）.There was no statistically significant difference in the incidence of adverse reactions between the two groups （ P >0.05）.Conclusion The application of dapagliflozin in elderly T2DM outpatients can reduce glycemic fluctuations，fectivelylowerblood glucose，bloodpressure，bloodlipids，blooduricacid，andimprovecardiovascular risk indicators，and has good safety.
