[Abstract]Patients with skeletal Class I hyperdivergent paterns often present with mandibular retrognathism,increasedlowerfacialheight,andexcessiveverticaldimensionofocclusion,resulting inanunaestheticfacialprofileanda higher incidence of temporomandibular disorder (TMD). Conventional treatments such as camouflage orthodontics with extraction orsingle-jawsurgeryoften struggletoachieveabalance between improvingfacial aestheticsandfunctional outcomes.Inrecent years,atreatment approachcenteredoncounterclockwise rotation (CCR)ofthe mandible has gained atention. By adjusting theocclusal plane and mandibular position,this approach not only improves mandibularretrognathismand optimizes facial proportions but also helps reduce joint loading and alleviate TMD symptoms to some extent. Thisarticle systematicallydiscusses theroleofCCR in seletal Class Ihyperdivergentcases from perspectives such as biomechanical foundations,orthodonticandorthognathic treatmentstrategies,joint functional adaptation,andclinicalapplications,while also offering insights into its stability and future research directions.
