Abstract Objective：To analyze the influencing factors of anastomotic leakage in gastric cancer patients after gastrectomy by systematic review. Methods： PubMed， Web of Science， EMbase， CNKI， WanFang Database， and VIP were searched for case-control and cohort studies on influencing factors of anastomotic leakage after gastric cancer surgery. The retrieval time was from the inception to February 2024. Two researchers independently screened literature， extracted data， and evaluated the risk of bias for inclusion in the study. Meta analysis was conducted using RevMan 5. 3 software. Results：A total of 17 articles were included，including 12 case-control studies and 5 cohort studies， with a total of 20 424 patients. The results of Meta-analysis showed that gender， age， BMI， combine diabetes mellitus， operation time， surgical method， respiratory damage， blood loss and level of preoperative hemoglobin were influencing factors for anastomotic leakage after gastrectomy in patients with gastric cancer. Conclusion：Current evidence shows that there are many influencing factors for anastomotic leakage in gastric cancer patients after gastrectomy. Clinical nurseys should closely monitor high-risk patients with ye levant influencing factors，so as to reduce the incidence of anastomotic leakage，improve the clinical prognosis of patients.
