【Abstract】Objective To evaluate the clinical efficacyof microscope-asisted anteriorcervical surgery in patients with ossificationof thecervicalposteriorlongitudinal ligament (OPLL)complicatedby spinalcord injury.Methods Atotalof 72 patients with OPLLwhounderwent anteriorcervicalsurgeryat Zhongshan HospitalAfiliated toDalian UniversityfromJanuary 2019to January 2O24wereretrospectively included.According tothesurgical aproachused,theywere divided intoa microscope-assisted group (n=38) )and a conventional group (n=34) ).Perioperative indicators, complications, and preoperative and12-month postoperative visual analogue scale (VAS)score,Frankel grade,JapaneseOrthopaedic Asociation (JOA)score, and JOA improvement rate were compared betwen the two groups.ResultsThe two groups were comparable in baseline characteristics,such as age, sex,type of ligament ossification,and number of involved vertebral segments (all P>0.05) ! Compared with theconventionalgroup,the microscope-assisted groupshowed significant advantages inshorteroperativetime, lessintraoperative blood loss,lower drainage volume,and shorter hospital stay (all P<0.05 ).At12 monthsafter surgery,VA score,Frankel grade,and JOA score were allimproved in both groups compared with those before surgery (all P<0.001 ).The microscope-assted grouphad beterVASscore,Frankel grade,JOA score,andJOA improvement rate than theconventional group (all P<0.05 ),anda lower incidence of complications( 2.6% vs. 11.8% ).Conclusions Microscope-assisted anterior cervical surgery hasadvantages in the treatment of OPLL complicated by spinalcord injury. Itcan reduce surgical trauma, decrease the incidence of complications, and improve surgical efficacy.
