Prognostic evaluation of prognostic nutritional index for patients with hepatectomy for primary hepatocellular carcinoma:a Meta-analysis

Abstract Objective:To systematically evaluate the relationship between prognostic nutritional index (PNI) and prognosis in patients with hepatectomy for primary hepatocellular carcinoma(HCC).Methods:A comprehensive retrieval of literature on the relationship between PNI and prognosis of patients with hepatectomy for HCC was conducted from PubMed,Baidu Scholar,CINAHL,Google Scholar,Web of Science,OVID,CBM,CNKI,WanFang Database and VIP.The retrieval time was from the inception to May 31,2022.Meta-analysis was performed using RevMan 5.2 software.Results:A total of 6 articles,including 1 459 patients were included in the Meta-analysis.There was statistically significant difference in OS between patients with low PNI and those with high PNI(HR=1.94,95%CI 1.53-2.44,P<0.001),RFS of patients with low PNI was significantly different from those with high PNI(HR=1.59,95%CI 1.34-1.89,P<0.001),subgroup analysis showed:the predictive value of PNI in HCC patients with hepatectomy was significantly different between Chinese population(HR=1.95,95%CI 1.53-2.48) and Japanese population(HR=1.96,95%CI 1.21-3.18),and PNI between 45.00 and 49.50 had better predictive effect.Conclusion:Current evidence shows that PNI levels were significantly correlated with OS and RFS in HCC patients undergoing hepatectomy,and low PNI could be used as an effective prognostic marker for such patients.
